The Maryland Health Care Commission (MHCC) and MedChi, The Maryland State Medical Society (MedChi), invite you to participate in an important discussion on advancing care coordination and optimizing reimbursement opportunities through recently adopted Medicare codes.
During this session, we will:
- Explore new Medicare reimbursement codes for Social Determinants of Health Risk Assessment, Community Health Integration, and Principal Illness Navigation
- Share strategies to establish effective communication, define care team roles, and implement systems for information sharing and patient follow-up across primary care, specialists, and other health care partners
- Demonstrate how to leverage CRISP and electronic health records (EHRs) to enable seamless data exchange and strengthen relationships with external providers
- Provide guidance on creating protocols that support closed-loop referrals
